Abstract

OBJECTIVE:

A major hurdle in osteoarthritis (OA) research is the lack of sensitive detection and monitoring methods. It is hypothesized that proteases, such as matrix metalloproteinases (MMPs), are up-regulated in the early stages of OA development. This study was undertaken to investigate if a near-infrared (NIR) fluorescent probe activated by MMPs could visualize in vivo OA progression beginning in the early stages of the disease.

CONCLUSION:

Imaging of MMP activity in a mouse model of OA provides sensitive and consistent visualization of OA progression, beginning in the early stages of OA. In addition to facilitating the preclinical study of OA modulators, this approach has the potential for future translation to humans.

Resumen

OBJETIVO:
Un obstáculo importante en la
 investigación de la osteoartritis (OA)  es la falta de métodos de detección y monitoreo sensibles. Se planteó la hipótesis de que las proteasas, tales como metaloproteinasas de la matriz (MMPs), están regulados en las primeras etapas del desarrollo de la OA. Este estudio se realizó para investigar si una (NIR) sonda fluorescente del infrarrojo cercano activado por MMPs podría visualizar in vivo la progresión de OA a partir de las primeras etapas de la enfermedad.

Conclusión
La obtención de imágenes de la actividad de MMP en un modelo de ratón de OA proporciona una visualización sensible y coherente de la progresión de la OA, comenzando en las primeras etapas de la OA. Además de facilitar el estudio preclínico de moduladores OA, este enfoque tiene el potencial para la traducción futuro a los seres humanos

Abstract


Previously published studies reported variable results using various suture techniques and reconstruction options for massive rotator cuff tears. Therefore, the current authors retrospectively studied 21 consecutive patients/shoulders with massive rotator cuff tears treated from January 2005 to October 2011 with a human dermal allograft through a mini-open approach. Mean patient age was 58 years (range, 33-72 years). Mean follow-up was 29 months (range, 18-52 months). Ten patients underwent revision repair for a failed rotator cuff repair. The authors measured the tendon gap (mean, 1.7 cm) and acromiohumeral interval (mean, 6.5 mm). They evaluated pain, shoulder range of motion (ROM) and function, patient satisfaction with the operation and outcome, and complications. At last follow-up, all patients experienced significant pain relief (P=.001) and improved ROM (P=.001) and American Shoulder and Elbow Surgeons (ASES) score (P=.001). Eighteen patients reported that they were satisfied or very satisfied and 3 reported that they were not satisfied with the operation and outcome. Comparison of outcomes between patients who underwent primary repair and those who underwent revision repair and between patients who had muscle atrophy and fatty infiltration grades 0 to II and those who had grades III to IV showed no statistically significant differences. A statistically significant correlation was observed between the size of the tendon gap and postoperative pain, ROM (except external rotation), and ASES score (P<.050). No significant correlation was observed between postoperative pain, ROM, and ASES score and the acromiohumeral interval (P>.050). Complications related to the dermal human allograft were not observed.

Estudios publicados anteriormente reportaron resultados variables utilizando diversas técnicas de sutura y las opciones de reconstrucción para desgarros masivos del manguito rotador. Por lo tanto, los autores del presente Estudio retrospectivo de 21 pacientes consecutivos / hombros con desgarros masivos del manguito rotador tratados entre enero de 2005 octubre de 2011 con un aloinjerto de dermis humana a través de un enfoque de mini-abierto. Abstract

OBJECTIVE:

The aims of this study were to (1) examine the pattern of experimental pain responses in the affected and nonaffected extremities in patients with shoulder pain and (2) explore the intraindividual association between sensitization states derived from experimental pain testing.

METHODS:

Experimental pain responses from 58 patients with shoulder pain (17 women, aged 18 to 52 y) were compared with those from 56 age-matched and sex-matched pain-free volunteers (16 women, aged 21 to 58 y). Experimental pain responses included pressure pain threshold (PPT), thermal pain threshold and tolerance, and suprathreshold heat pain response. Comparisons were made between the affected and nonaffected extremities of clinical participants and the average response of extremities in control participants. Peripheral and central sensitization indexes were computed for clinical participants using standardized scores and percentile cutoffs on the basis of the data from the control sample. Experimental pain responses in clinical participants observed beyond the 25th and 75th percentile of control sample responses were used for investigation of intraindividual association of sensitization states.

RESULTS:

PPT at the acromion and masseter on the affected side of clinical participants were diminished compared with that on their nonaffected side (P<0.015). Bilateral sensitivity in clinical participants was noted for PPT at the acromion and suprathreshold heat pain response (P<0.015). Peripheral and central sensitization indexes demonstrated that individuals with shoulder pain present with variable patterns of peripheral and central sensitization.

CONCLUSIONS:

Collectively, experimental pain responses supported peripheral and central sensitization in response to pressure and thermal stimuli. No clear association was made between individuals exhibiting peripheral or central sensitization, thus suggesting heterogeneity in pain processing in this clinical population.

Sensibilización central y periférica en el dolor de hombro

La influencia de los procesos de sensibilización central y periférica en la percepción del dolor, recuperación y desarrollo del dolor crónico están empezando a recibir atención en la investigación clínica. Este estudio, utilizó varios test experimentales de sensibilidad al dolor para investigar si la respuesta al dolor en el lado afecto y no afecto de pacientes con dolor de hombro, difería de los casos control asintomáticos.
Se incluyó en este estudio a 58 pacientes con dolor de hombro y 56 pacientes control libres de dolor y emparejados por edad y sexo. Todos los sujetos completaron cuestionarios estándar de características demográficas y psicológicas (cuestionario de miedo y dolor, escala de pensamientos catastróficos frente al dolor y escala Tampa de kinesiofobia)  antes de los test. Los test utilizados incluyeron valoración de la sensibilidad dolorosa a la presión y térmica.
Se registraron evidencias de sensibilización periférica (p.ej. diferencia de un lado al otro en sensibilidad a la presión) así como la evidencia de sensibilización central (p.ej. presión bilateral e hipersensibilidad térmica en regiones locales y remotas en comparación con los sujetos asintomáticos) en pacientes con dolor de hombro unilateral. No se encontró evidencia de patrones consistentes de sensibilización en esta población, lo cual indica una heterogeneidad en el procesamiento del dolor en las patologías musculoesqueléticas.
